What Documents Are Required to Work in Each State with ESHYFT?

If you would like to add additional states to your profile, you must meet that state's compliance requirements before you can pick up shifts there.

General Requirements for All States

All applicants must provide:

  • Profile Picture

  • License Certificate

  • Driver's License or Passport


TB Screening

All states require proof of TB screening completed within the past 12 months.

The following are accepted for a TB Test:

  • TB Skin Test (1-Step or 2-Step where applicable)

  • QuantiFERON or other TB Blood Test

  • Chest X-Ray (valid within 5 years)

States Requiring a 1-Step TB Test

AL, AR, CT, FL, GA, IA, IL, IN, KS, KY, MD, MI, MT, NC, NE, NY, OH, RI, TN, VA, WA, WI, WV

States Requiring a 2-Step TB Test

DE, MN, MO, NJ, PA, VT


Physical Exam

A physical examination is required in the following states:

DE, MA, NJ, NY, PA, WV

Physicals must:

  • Be completed within the past 12 months

  • State that the applicant is cleared to work without restrictions


CPR / BLS Certification

Required for all LPNs and RNs

Required for CNAs in DE


Dementia Training

Delaware

Required:

  • 4-hour Dementia Training completed within the past 2 years

  • 4 additional hours required annually

Indiana (IN)

Optional:

  • 6-hour Dementia Training Certificate


Dependent Abuse Training

Iowa (IA)

Required:

  • Dependent Abuse Training Certification (Good within 3 years)


COVID-19 Vaccination or Exemption

Required in Maine and Rhode Island


MMR Vaccine or Titers

Required in:

  • ME

  • MD

  • NY

  • RI


Varicella Vaccine or Titers

Required in:

  • ME

  • MD

  • RI


Tdap Vaccine or Titers

Required in:

  • MD

  • RI


Hepatitis B Vaccine

Required in:

  • ME


Residency & Fingerprinting Requirements

Applicants who have not lived in Pennsylvania for the past 2 years must complete FBI fingerprinting through the Department of Aging.


Please note that once we receive all required documents, additional state-specific verification checks may be required. Third parties process these verifications and may take anywhere from a few days to several weeks to complete before you are approved to work in that state.
