Licenses expire on the ESHYFT app, and when they do, we will need your updated documents to keep your account active.
Please make sure your updated license and any required documents are ready before your current license expires.
When you are ready, email [email protected] with your updated documents to complete your update.
What Documents Do I Need?
All healthcare professionals must submit:
Updated License or Certification
Valid TB Test within the past 12 months
A Two-step TB test is required in the following states: DE, MN, MO, NJ, PA, SC, VT
All other states require a one-step TB test
Additional requirements vary by state.
Physical Exam
DE, MA, NJ, NY, PA, WV
CPR Card
AL, DE, FL, IA, IL, IN, MA, ME, MI, MO, MT, NC, OH, PA, RI
Please note: TB tests and Physical Exams must have been completed within the past 12 months to be accepted.
