for you and up to 8 dependents!

**Eligibility is currently limited to New Jersey and Minnesota**


What’s Included?

All benefits are for you and up to 8 dependents

Enrolling

  1. Submit this one-time form to enroll.

  2. Work 10 shifts before the 25th of each month.

Shifts are counted from the 1st to the 24th each month. Coverage starts on the 1st of the next month and lasts through its end. Completing 10 shifts per cycle keeps your subscription active. If you don’t qualify, coverage ends that month and resumes automatically once you qualify again.


Telehealth App Activation

After completing the form and working 10 qualifying shifts, you will receive an email to activate your Telehealth account.
You can activate your account using one of these 3 methods. Use the email address you have on file with ESHYFT to complete your registration.

Adding dependents to your subscription

Add your dependents as soon as possible in these 2 places.
Your member number is in the email you received from BasiCare.

Reactivating your subscription

When requalifying (by working 10 shifts in a future month), your subscription will automatically reactivate. Simply log back in to the Recuro app to continue using your benefits - there is no need to re-enroll with the form or re-activate with BasiCare.

Is This Health Insurance?

No. BasiCare is NOT Insurance. It is a telehealth subscription meant to be used in non emergency situations. BasiCare Plus® and its Rx benefit are NOT INSURANCE. Use of telemedicine is strictly for non-emergency general illness.


Important Notes on Eligibility

*Timecards must be submitted by 11:59 PM on the 24th to qualify. Timecards must be worked and submitted via the ESHYFT app. Manually created or rejected timecards do not qualify. Timecards must be submitted before the cut-off date/time. Non eligible timecards include, but are not limited to, manually created timecards such as timecards created by Missed Punch Forms, or worked shifts not clocked in and out using the ESHYFT app, and Rejected or fraudulent timecards. A timecards eligibility is at the sole discretion of ESHYFT.
