What if the app crashes before or during my shift?

Please obtain written approval from your facility manager if you experience any technical difficulties on the app during your shift.

Technology is tricky. If you experience any technical difficulties with the app during your shift, please still complete your scheduled shift and do the following so we can pay you for the hours worked. 

  1. If you are able to print, fill out and get this document signed before attaching it to the form below.

    If you are unable to print the form, please submit either a screenshot of an email from the facility manager with their email address visible, or submit a piece of paper with the date, time, and signature of the facility manager.

All timecard adjustments need to be submitted by 4pm on Sunday, otherwise, you risk delaying your pay if the form is submitted any later.
