We get it - you show up to a facility to work and you forget to clock in, or you're so tired or in a rush to get home that you forget to clock out. In the event you forget to clock in or clock out, you can submit a timecard adjustment form.
If you are able to print, fill out and get this document signed before attaching it to the form below.
If you are unable to print the form, please submit either a screenshot of an email from the facility manager with their email address visible, or submit a piece of paper with the date, time, and signature of the facility manager.
All timecard adjustments need to be submitted by 4pm on Sunday, otherwise, you risk delaying your pay if the form is submitted any later.
